Directions for office in Manhattan:

  • Our entrance is located on Second Avenue between East 17th and East 18th street. Once you enter the building, turn left.
  • Proceed to this small alcove. Once inside the alcove, go through door on the right.
  • Turn right (you will see mail boxes ahead). Proceed to the mailboxes.
  • Make a quick left where you will see the elevator. Take elevator to level C.
  • Turn right as you exit the elevator, you will see this sign
  • Turn right and straight ahead will be a hallway which you will go to the end. Make a quick right.

Please arrive 10-15 minutes early for your Appointment for registration and to fill in a health questionnaire.

When you come for your Appointments, please remember to bring the following

  • Drivers License or a valid ID
  • Insurance information
  • Referral Letter (if required)
  • Copies of results, x-rays, MRI's. CT scans etc and any other relevant information
  • List of medications (if any)

To cancel an Appointment

Telephone the office during business hours and allow at least 1 day's notice so that we can offer your appointment time to patients on our waiting list.

We recognise that your time is valuable, and we make every effort to run on time. Occasionally emergencies or patients require a little more time, and these cause scheduling delays beyond our control. We apologise if we keep you waiting.
